Hi,

For these both modules, I lost some layout.
Contact Us: grey background
Call us: icon and border.

And the "contact us" module is just for example. It is not working like a contact form!

The contact form is a demo only and its not working in Joomla because Joomla has his own Contact component.
But you can install a form extension where you can add a contact form to any module position.
