I made the below link work once before but I'm not sure how I did it and now I can't get it to work again. It links to a page that contains a form. When it worked, it opened on a new page and looked great. I have no idea what I did to make it quit working but now all I get is a blank page. I also have this page in a menu as hidden. I've tried using the page link as the Href as below and I've also tried using the Alias but neither work:
